What this inquiry form collects
Your adult contact name, email, request category, class and membership preferences, adult confirmation and optional marketing choice are stored to handle your request. A request reference and follow-up record help prevent duplicate submissions. The application uses a hashed request-rate identifier to limit abuse.
This website is directed to parents, guardians and adult athletes. Do not submit a child’s name, date of birth, health information, private film, school documents or card numbers. This release has no youth record entry or uploads.
How this release uses the information
Adult inquiries are stored in DTU’s website database on the Sites hosting infrastructure. They are available to authorized site operators for follow-up. This release does not send inquiry data to D1, an advertising platform or an email provider. Email delivery is not connected. A saved request does not mean an email was sent.
No advertising pixels, session replay or additional analytics cookies are added by this application. Hosting providers may process technical request information to operate and secure the site.
Your choices
Marketing permission is optional and separate from a response about your request. You can request correction, deletion or withdrawal of marketing permission using the privacy category below. Inquiries remain stored until an authorized operator handles them; this release does not run automatic deletion. Do not include sensitive details in a privacy request.
